2015 Renewal  

             All Renewals Begin August 1, 2014

   Fire Extinguishers &Fire Suppression November 1, 2014

   Sprinklers December 1, 2014

 

RENEWALS

FEP=Portables, PEK=pre-engineered kitchen, PEI=pre-engineered industrial and ESP=engineered fire suppression licenses/permit:

 

                  DOCUMENTS NEEDED:

 

1.      Company letter outlining your request in detail ( include any changes); state any changes

2.      Annual Registration with Georgia Secretary of State Corporate Division

3.      Copy of Insurance certificate 

4.      Color jpeg of tags, collars and labels

5.      Separate company letter stating who you subcontract hydro-static testing if you do not do this service

6.      Applicable fees for each license type and permit renewal (not for profit/ registrations do not require a fee)

7.      If renewal is not U. S. citizen include Citizenship form with document

8.      Be sure to put date of last Facility and or mobile inspection by our office

9.      Copy of CEU certificates and certification certificates for each technicians permit

10.   Copy of check and company letter included with scanned documents

13.   Mail the company letter and check to lockbox

SPRINKLER RENEWALS

DOCUMENTS NEEDED:

     
  1. Company letter outlining your request in detail including any changes from last issuances.
  2. Copy of check and company letter included with renewal scan ( mail only letter and check to lockbox) 
  3. Insurance certificate
  4. Certification for Certificate of Competency ( NICET LEVEL III OR BETTER IN LAYOUT)
  5. Inspectors certification (if renewing)
  6. If renewal is not U. S. citizen include Citizenship form with document
  7. Testing information from NICET  in inspections and testing or letter showing next test date (if renewing)
  8. Designer license certification certificate ( current PE LICENSE in GA or NICET LAYOUT  level II cert. ) (optional renewal)
  9. Applicable renewal fees for each license renewal
  10. Color photo of tags in one photo: three up and three down in color photo ( must be clear on zoom)
  11. Scan document in one PDF file and email to sclark@sfm.ga.gov
